Standard Report - OSHA Form 300

Required Setup

Pulls only those WcCaims which has date type as Injury Date in date section.  The Year ID filter pulls the date from the Injury Date/Time field.

Report Fields

Field Name

Explanation

Year

Pulls the year of the parameter.

Establishment Name

If parameter Report Based On is selected as Supplier, Establishment Name pulls staffing supplier description.

If parameter Report Based On is selected as Branch, Establishment Name pulls site description.

In case of parameter Report Based On selected as Customer:

If Roll Up To Root Customer is True, Establishment Name pulls "root customer name".

If Roll Up To Root Customer is False, Establishment Name pulls "root customer name - customer department name".

City

Address: Displays the addresses based on the parameter Report Based On (Supplier’s, Branch’s, Customer’s): Pulls City.

State

Address: Displays the addresses based on the parameter Report Based On (Supplier’s, Branch’s, Customer’s): Pulls State.

Case No.

Shows WCclaimID from WCClaim.

Employee Name

Pulls employee name from employee table.

Job Title

Pulls Assignments' Job Title.

Date of Injury or Onset of illness

Puls date from date type “Injury Date” with the category of “WcClaimDate”.

Where the event occurred

Pulls AccidentDescription from WCClaim.

Describe the injury or illness, parts of body affected, and object/substance that directly or made person ill

Pulls all body parts injuries + notes in comma separate from WcClaimBodyPartInjured.

Death

Shows as checked if the extra value for systemname “MostSeriousOutcome” has been set up in WcClaim extra section.

Days away from work

Shows as checked if the extra value for systemname  “MostSeriousOutcome” has been set up in WcClaim extra section and extra value is “Days Away from Work”.

Job Transfer or restriction

 

Shows as checked if extra  value for systemname  “MostSeriousOutcome” has been set up in WcClaim extra section and extra value is "Job Transfer or Restriction"

Other recordable cases

Show as checked if the extra value for systemname “MostSeriousOutcome” has been set up in WcClaim extra section and extra value is  "Other Recordable Cases".

Away from work

Shows extra value if the extra value for systemname  “No.oflostdays” has been set up in WcClaim extra section.

On job transfer or restriction

Shows extra value if the extra value for systemname  “No.ofOnJobTransfer” has been set up in WcClaim extra section.

Injury

 

Shows as checked if the extra value for systemname  “InjuryType” has been set up in WcClaim extra section and extra value is “Injury”.

Skin Disorder

Shows as checked if the extra value for systemname  “InjuryType” has been set up in WcClaim extra section and extra value is “Skin Disorder”.

Respiratory

Poisoning

Hearing Loss

All other illness

Shows as checked if the extra value for systemname  “InjuryType” has been set up in WcClaim extra section and extra value is “All Other Illnesses”.
